【抑郁症】针刺与艾灸,谁能还给你那被偷走的快乐?

“当神经细胞出了问题,常人可以承受的刺激在抑郁症病患脑中却会引起严重的负面情绪。指责抑郁症患者不坚强,和指责聋哑人不说话一样,是没有道理的。”



伴随着经济的高速发展和生活工作节奏的加快,抑郁症侵入了现代人的生活,严重影响了现代人的精神状态和生命健康。如此一来,寻找有效途径克制抑郁症已是刻不容缓。可喜的是,相关研究已经证实针刺与艾灸对抑郁症有确切疗效且副作用小。于是,很多人开始探讨针刺与艾灸在治疗抑郁症方面,疗效是否存在差异?这一场较量,到底是势均力敌还是强弱悬殊?


With the rapid development of social economy and the accelerated pace of life and work, depression has invaded the life of modern people, seriously affecting the spirit and health of modern people. Hence, it is urgent to find effective ways to control depression. Fortunately, some related researches have proved that acupuncture and moxibustion therapy have definite effects on depression with fewer side effects. So, many people begin to discuss whether there is a difference of the effects on depression between acupuncture and moxibustion. In this contest, is it a fine match? Or is there a wide disparity in their strength?



为了验证针刺与艾灸的疗效是否存在差异,研究员利用老鼠来做实验,比较二者对慢性不可预见性温和应激(chronic unpredictable mild stress, CUMS)抑郁模型大鼠行为学、前额皮质5-HT系统(抑郁症与中枢单胺类神经递质5-HT等下降或缺乏从而导致中枢5-HT系统功能紊乱有关)及5-HT1A受体功能(抑郁症存在突触后膜5-HT1A受体低敏等病理改变,从而导致5-HT神经传递功能下降)的影响。


In order to verify whether there is a difference of their effects on depression between acupuncture and moxibustion, he researcher uses mice to do the experiment to compare the effects between acupuncture and moxibustion on ethology and the function of 5-HT system (the occurrence of depression is related to the dysfunction of central 5-HT system caused by decreased or lack of central monoamine neurotransmitters, 5-HT and others) and 5-HT1A receptor (patients with depression have pathological changes including low-sensitive postsynaptic 5-HT1A receptors and others, resulting in the decline of serotonergic neurotransmission function) in the prefrontal cortex in rat model of depression induced by chronic unpredictable mild stress (CUMS).



研究员选取SD雄性大鼠40只,并根据体质量将动物随机分为正常组、模型组、氟西汀组、针刺组、艾灸组,每组8只。正常组每笼四只,其余各组则单笼饲养。采取不同的方法,如4℃冰水游泳,45℃热水游泳,明暗颠倒24小时、禁水24小时、禁食24小时等,对大鼠进行刺激,为期5周,对除正常组外的各组大鼠进行CUMS模型制备。由于快感缺失是抑郁症的核心症状,因此研究员采用糖水消耗实验计算糖水消耗百分比作为评价快感缺失的客观指标,同时采用射免疫法检测前额皮质5-HT、5-HIAA、Trp含量。然后,对不同组的大鼠采取相应的治疗方法。针刺组和艾灸组对大鼠的“百会”和“大椎”穴位分别进行针刺和艾灸,每日1次,每次20 分钟,持续3周。


The researcher selects 40 male SD rats and randomly divides them into normal control, depression model, fluoxetine, acupuncture, and moxibustion groups, eight rats in each group. Rats in normal control group are reared 4 per cage, while each rat in other groups is reared alone. Then, the researcher takes different ways to stimulate those rats, such as cold water swimming at 4°C, hot water swimming at 45°C, reversed dark/light cycle for 24h, water deprivation for 24h, food deprivation for 24h and so on. Except rats in the normal group, rats in other groups are used for the preparation of CUMS model for a total of five weeks. Anhedonia is the core symptom of depression, so sucrose consumption test is used and the percentage of sucrose consumption is calculated as an objective evaluation indicator for anhedonia. At the same time, the content of 5-HT, 5-HIAA, Trp in the prefrontal cortex tissue is assayed by radioimmunoassay. Later, taking appropriate ways to cure different groups of rats. In the acupuncture group and the moxibustion group, researcher uses acupuncture or moxibustion stimulation of “Baihui” and “Dazhui” for once a day and 20 minutes each time. The treatment will last for three weeks. 



通过实验,研究员发现,造模14天后模型组及各治疗组糖水消耗百分比较正常组均显著下降,而治疗3周后,各治疗组糖水消耗百分比较模型组均显著升高。这说明,治疗3周后,针刺与艾灸均能够很好地改善模型大鼠快感缺失的抑郁状态且无明显差别,疗效与氟西汀相当。并且,通过观察5-HT、5-HIAA、Trp的含量,发现模型组大鼠存在5-HT分解代谢增强、Trp利用率下降,5-HT系统功能减低,5-HT1A受体亲和力和数目的下降,针刺与艾灸则能够显著减低5-HT的分解代谢水平,显著提高Trp的利用率,并提升5-HT1A受体的亲和力和数目,改善突触后膜5-HT1A受体低敏的趋势,疗效与氟西汀相当。


Through the experiment, the researcher finds that fourteen days after the model establishment, compared with the normal group, the percentages of sucrose consumption in the model group and treatment groups significantly decrease; but after three weeks of treatment, compared with the model group, the percentages of sucrose consumption in treatment groups significantly increase. It indicates that acupuncture and moxibustion are able to greatly improve anhedonia in the rat depression model and there is no significant difference in the efficacy between acupuncture and moxibustion after 3 weeks of treatment. 


And the efficacy of acupuncture and moxibustion is similar to that of fluoxetine. In addition, through the observation of the content of 5-HT, 5-HIAA and Trp, it can be found that rats in the depression model group have increased catabolism of 5-HT, decreased utilization rate of Trp, reduced function of 5-HT system and the affinity as well as the number of 5-HT1A receptor, while acupuncture and moxibustion can significantly reduce the level of 5-HT catabolism, significantly improve the utilization rate of Trp, increase the affinity as well as the number of 5-HT1A receptor and improve the low-sensitive trend of postsynaptic 5-HT1A receptor, similar to the effects of fluoxetine.

 

总体来看,这场关于疗效的较量,针刺与艾灸势均力敌,疗效相当;调节5-HT系统及5-HT1A受体功能紊乱可能是针刺与艾灸抗抑郁的作用途径之一。


Overall, this contest between acupuncture and moxibustion is a fine match and their effects on depression are similar; regulating 5-HT system and the dysfunction of 5-HT1A receptor may be one of the mechanisms of acupuncture and moxibustion treatments on depression.
