This patient had a long-standing history of severe chronic sinusitis. In addition to extensive diffuse inflammatory mucosal thickening involving the paranasal sinuses, nasal polyposis, and surgical changes of the paranasal sinuses and the nasal cavity, we see expansion of the frontal sinuses with T2/FLAIR hyperintense contents, which are additionally noted to have some intrinsic T1 hyperintensity on non-contrast enhance images, lightly reflecting a heterogeneous mixture of proteinaceous content.  

The frontal sinuses are expanded，more so on the right, and the walls the sinuses are significantly thinned. A separately performed CT not shown here did not demonstrate gross osseous dehiscence. Post contrast images demonstrate mucosal enhancement within the frontal sinuses, but no mass like enhancement associated with the sinus expansion. 

These findings are consistent with bilateral frontal mucoceles. Mucoceles arise from obstruction of the sinus ostium and continued secretion of mucus. Pressure necrosis is thought to contribute to the classic associated sinus expansion. The most commonly occur in the frontal sinuses and can lead to orbital compression as in this case. 

MR can be utilized to evaluate for a possible underlying obstructing neoplasm or intracranial extension. This patient went on to have successful endoscopic sinus surgery.

【Frontal Mucocele 额窦黏液囊肿】
这位患者有长期严重的慢性鼻窦炎病史。此外还有鼻旁窦黏膜广泛弥漫的炎性增厚和鼻息肉病，鼻旁窦和鼻腔呈术后改变。非对比增强图像显示额窦扩大，其内可见T2/FLAIR高信号、T1极高信号的内容物，提示不均匀的蛋白成分。
额窦向右边扩大，窦壁明显变薄。单纯的CT图像未给出，所以不能显示整体的骨质断裂。对比增强检查显示额窦黏膜强化，未见肿块样强化和窦腔蔓延。
以上征象符合额窦黏液囊肿的表现。黏液囊肿是由于窦孔阻塞，并分泌黏液积聚而成。典型的窦腔扩张导致可导致周围组织受压坏死。最常见好发于额窦，这个病例中可见眼眶受压。
MR是评估潜在的梗阻性肿块和腔内蔓延情况的最佳方法。此患者成功行窦道内镜手术。
